Job description

Salary Range £27,416 to £33,779 pro rata (depending on experience). 25-30 hours worked over 3 or 4 days. Current NHS pension scheme members can continue in the scheme under LOROS’ direction body status and NHS service will be taken into consideration in relation to annual leave entitlement

The opportunity has arisen to join our Discharge Team. You will be responsible for:-

• The safe and effective discharge of patients, in line with current legislation. Facilitating discharge to home or nursing home working alongside the multi-disciplinary team. • Continuing Healthcare assessments, including the completion of CHC checklists. • Requests for packages of care which evidence the needs of the patient.

The successful applicant will be an experienced nurse with current NMC registration and possess excellent communication skills. An understanding of continuing healthcare; the discharge process; and an awareness of relevant legislation (e.g. the Mental Capacity Act 2005) is essential.
