- Medical History
How will we use the information about you?
This information is used for your treatment whilst you are receiving care from LOROS either at your home, within the hospice or Manor Croft Day Centre. The information you provide us will be securely retained in a patient medical record file. This information will also be entered on to our Patient administration system called HCAS.
We have an associated company called LOROS Lottery and internally we have a fundraising team. We may share information with these departments relating to your name, address and contact details. This will only be shared with them if you make any donations to LOROS and you consent for them to record this information on their computer databases. Information regarding your next of kin may be shared with them if your circumstances change.

What information do we collect about you?
- Name
- Address
- Date of Birth
- Telephone Number
- Email Address
- Amount of any donation and whether you have registered for Gift Aid
- Fundraising Events you attend
How will we use the information about you?
We will use this information to send you an acknowledgement letter to thank you for your donation.
The information collected will be added to our computer database (Raiser’s Edge) so that we can keep you updated on any future fundraising activities, events and the hospice care services, unless you tell us otherwise.
We will share this information with Her Majesty’s Revenue and Customs (HMRC) if requested to do so.

Information entered onto Lottery database is used solely for the lottery draw.
Bank details are used solely for the collection of subscriptions for lottery draw
We will share the above information with Sterling* who administer the draw on behalf of LOROS Lottery, to comply with the Gambling Act.
We may share your information with a third party company* who will act on our behalf for the annual Lottery Bumper Draw.
We will share the following information with our associated charity LOROS, who will provide you with information about its care services and events for as long as appropriate under data protection, or unless you advise us not to do so:
